Pricing and Chargemaster Information

In compliance with Florida state law, Section 395.301, F.S., Florida Rehabilitation Hospital at Tampa provides transparency in our pricing for our services, which are service bundles with non-personalized estimates of charges, and we provide the process for patient billing to all of our patients.

 

Patients and prospective patients have the right to request a personalized estimate from the hospital.

To improve price transparency, all U.S. hospitals and health systems are required to provide lists of standard hospital charges which is also called a chargemaster that has all of our charges for our services to facilitate comparison of prices. However, here are a few considerations to keep in mind as you view the list of standard charges.

Please note that physician services are not included and are billed separately by each individual physician and paid separately by the respective insurance plan. The information provided here does not include any physician services

.

Occasionally other services may be required and services may be billed by a different provider. Examples of these additional services may include but are not limited to:

  • Special equipment

  • High cost pharmacy items

  • Dialysis treatments

  • High cost diagnostic services such as a CT Scan or MRI

 

  • These charges aren’t typically the price that patients pay. The chargemaster lists the amount set for each service before insurance contract benefit plan discounts, so the price patients pay tends to be less than the standard charge.

  • Hospital charges are different from patient to patient for the same service depending upon the level and frequency of treatment.

  • Patients who are eligible for financial assistance may receive additional discounts.

  • Items included in a charge may vary across our hospitals.

  • Our various hospital charges are separate from the high-quality care and service we provide

The list may be useful for comparing pricing with other hospitals, but the list does NOT indicate actual costs to patients. Charges based on diagnosis are a non-personalized estimate of costs for anticipated services, however, actual costs will be determined based on the services actually provided. If “NA” is listed or the section is blank under the plan you are researching, you will need to scroll down the column to find the relevant pricing category for that particular plan. The Centers for Medicare and Medicaid Services (CMS) published a listing of 70 Shoppable Services listed with their primary billing code(s) which are required for hospitals to display their negotiated charges by each managed care plan. If a particular service is not offered by our hospital, an “NA” will be displayed. The required services are included below in the Shoppable Services file.

Financial Assistance, Billing, & Collections

For those who do not have the funds to pay the rehabilitation hospital bill, financial assistance may be available. Please review our financial assistance policy for information regarding financial assistance, billing, charity care, and collections. Please complete the following application for consideration.

If you do not have health insurance, we provide financial assistance for medically necessary care as a discount from our normal charges if your household income does not exceed four times the Federal Poverty Guidelines and you are a US Citizen. All applicants will be screened for Medicaid coverage and must cooperate with the Medicaid representatives to be considered for financial assistance. If you are eligible for financial assistance under our Policy, you will receive free or discounted assistance according to the following income criteria:

  • If your annual household income is up to 200% of the Federal Poverty Guidelines, you may qualify for free care.
     

  • If your annual household income is between 201% and 400% of the Federal Poverty Guidelines, you may receive care discounted to the amount we generally bill insured patients for such services.

 

Even if you have insurance, as long as you meet our income criteria, you may be eligible for financial assistance if: your insurance does not provide coverage for the medically necessary services you are seeking or you have exhausted your lifetime maximum insurance benefits. Financial Assistance cannot be used to cover deductibles or coinsurance for your insurance plan.

Additional Ways to Qualify

If you do not meet the income criteria above, you may be considered on a case-by-case basis for financial assistance under the following circumstances:

  • Catastrophic Balance: If you will have a balance due to Florida Rehabilitation Hospital at Tampa of greater than 25% of your annual household income, you may be considered for financial assistance.
     

  • Special Medical Circumstances: If you are seeking treatment that can only be provided by Florida Rehabilitation Hospital at Tampa or you would benefit from continued medical services from Florida Rehabilitation Hospital at Tampa for continuity of care, you may be considered on a case-by-case basis for financial assistance for that specific treatment.
     

Charges Will Not Exceed Amounts Generally Billed

If you receive financial assistance under our Policy, you will not be charged more for medically necessary care than the amount we generally bill patients having commercial insurance or Medicare coverage.

Financial Assistance Application Form and Process

To review your eligibility for financial assistance, please complete and send the following information:
 

  • a completed financial assessment form
     

  • a copy of last year's tax return or your most recent bank statement
     

  • a copy of your most recent pay stub or unemployment check
     

  • proof of any state or federal assistance such as food


These items are necessary to document your financial conditions and for us to process your request for assistance.

Application for Financial Assistance must be completed and approved prior to admission. Our team can assist and advise you in the completion process.

Any Financial Assistance Application whether completed in person, online, delivered or mailed in, will be forwarded to the Financial Services team for evaluation and processing.

Patient Billing and Collections

Florida Rehabilitation Hospital at Tampa strives to work with every patient that does not qualify for financial assistance, to resolve unpaid balances.

  1. Mail at least 4 statements to the patient/responsible party to the address on file.

  2. Attempt to contact the patient/responsible party by phone.
     

Private Pay: PLEASE NOTE that this excludes the preadmission prompt pay requirement for private pay patients. For any patients needing to privately pay for services, payment is due prior to services being rendered or make a deposit with a Payment Agreement.
